commit (head): rest of the escapeURL()/decodeURL()

Final touches to UT_UTF8String::escapeURL()/decodeURL(). AFAICT
escapeURL() now conforms to XML Linking Language (XLink) Version 1.0,
section 5.4 (http://www.w3.org/TR/xlink/#link-locators).

I have made changes to the AW and HTML exporters that ensure href
attributes are always escaped, and added url decoding to the code that
feeds url's to the status bar.
