Win32 owner drawn menus (again)

    These have made a lot of good progress. I have a few
    observations and requests, though:

    *) Can you use 12x12 icons instead of 16x16? The gaps
    between the menus still looks too large

    *) Can you not use icons for menu items that have a
    state? Like:

    Align->Full, Left, Right, Center
    Formatting->Bold, Underline, ...

    There are 2 ways to do this:

    1) Use different icons to show different states (i.e.
    depressed icons)
    2) Use checkboxes

    For the record, I *strongly* prefer #2.
